I expressly authorize LaSalle General Hospital to contact and obtain information from all references, employers, public agencies, licensing authorities and educational institutions to verify accuracy of the information provided in the application, resume’ and job interview, unless otherwise stated above. I understand that if I am hired by LaSalle General Hospital, I am free to resign at any time, with or without prior notice, and LGH reserves the same right to terminate my employment at any time, with or without cause and without prior notice.
